You could argue that you don't need a separate page for practical skills, if you've done a thorough production diary - won't all the evidence be on that page?? We suggest you think of it from an assessor's point of view - as an assessor, who has to mark work based on specific criteria, wouldn't you prefer to have everything laid out clearly for you?
You could think of this a 'highlights' page - you're taking the best evidence from your production diaries and clearly presenting it for assessment of your practical skills...
